The Tomahawk Land Attack Missile (TLAM) can strike high-value or well-defended land targets. The missile was first deployed in combat in 1991. The latest variant, "Tomahawk Block IV," introduced in 2004, can use an onboard camera to transmit damage information or target photos before the strike. It has the ability to "loiter" in the target area to respond to new targets; a bi-directional satellite data link allows for flight-time reprogramming to any of the 15 pre programmed backup targets or any GPS target coordinates. It is equipped with a 1,000-pound conventional warhead, the range more than 1000 miles, Tomahawk missile deployed from ships or submarines.

Tomahawk Block IV uses GPS navigation and satellite data links to continue on a preset course. The missile can be reprogrammed to a new target during flight. Two-way satellite communication is used to perform mission changes throughout the flight. An onboard camera provides the commander with images of the target before the strike. The guidance system is assisted by terrain-contour matching (TERCOM).
The digital scene matching area correlation (DSMAC) system or GPS provides terminal guidance. The tactical Tomahawk weapon control system (TTWCS) integrated into the ship's system calculates the path for engaging the target. The system can plan new missions on the launching ship. TTWCS is also used to communicate with multiple missiles to reassign targets and redirect missiles in flight.

A Los Angeles class submarine launch three Tomahawk Block IV missiles near the sea of Persian Gulf, the mission is air strike Two TPQ 53 anti-artillery radar systems and one x-wavebands mobile air-defence radar system 900 miles away in desert.

Two TPQ53 radar systems distance twenty miles away, the second TPQ53 radar distance X-wavebands radar 30 miles away. So ,if you want to blow up three radar systems need three Tomahawk missiles, each missile cost more than 1 million US dollar. Total 3 millions dollars.

The AN/TPQ-53 radar system adopts advanced active phased array technology , it is a mobile anti artillery radar system with rapid response capability . It can identify, detect, and track various projectiles flying in the air, and quickly calculate the opponent's position for counterattack. The radar system is installed on two FMTV medium tactical trucks. Vehicle 1 carries the radar and main generator, vehicle 2 carries the control center and backup generator

I estimate the empty weigh of TPQ53 radar from 2 tons to 3 tons,(not include the trucks) ,it no need take 450kg warhead blow up a medium size radar system.
A X-waveband air-defense radar empty weigh hundreds of kilograms.
It just need consume a 150kg TNT or RDX explosive can destroy a TPQ53 radar, if the 450kg warhead divide into separate parts, each part contain 150kg explosives, its warhead can destroy or wreck three target.

The new design add a artificial intelligence module, and the integrate warhead divide into three separate bombs, each bomb contain 150 kilograms high conventional
Explosive. The AI module control the Tomahawk missile speed of fly, course, The time drop the bomb, how many bombs it drops, calculate speed of winds,
lead lag control , AI module make the bomb hit the target’s radar system accuracy ,

And the smart cruise missile attack the first ,the second TPQ53 radar systems , in first and second attack smart cruise missile play a role as a little bomber, it drops a bomb weigh 150kg above the 30 meters high altitudes . it drops one bomb attack one TPQ53 radar each time.
At the end,the third attack AI cruise missile carry the last 150kg warhead suicide hit X-waveband radar system and blow it up. This mission complete,
That’s said, one AI cruise missile three flight phase autonomously., attack three radar systems, and destroy three targets.

According to relevant reports, Tomahawk Block IV missile perhaps use a millimeter wave active radar guidance head, which locates target by emitting millimeter wave signals and receiving reflected signals. As smart cruise missile drop the first bomb attack first TPQ53 radar, it open a active radar, search and locate next target, Al through the next object is not fixed positions ,it mobile. Collaborate with global positioning system (GPS), THE smart missile easy search and find the second ,third target.

The Sky-M radar consists of 3 separate radars, each mounted on an independent car chassis, with a maximum speed of 80 km/h and a range of 1000 km. The system can process centimeter, decimeter and meter wave signals. The radar can operate independently, but also take joint action, using radar modules in various bands to give full play to its comprehensive advantages to find targets such as stealth aircraft and cruise missiles. The Sky-M radar can detect flying targets at an altitude of 75 km and a maximum distance of 600 km.

The Sky - M systems of Russia ,I estimate radar antenna empty weigh 6 tons or 7tons,, sky-M belong a big size radar system. If destroy the antenna of SKY-M radar system estimate need a 300kg explosive warhead. As a result, the AI cruise missile calculate the weight and size of SKY-M radar ,and drop two bombs(each bomb 150kg RDX explosive) attack the radar .

After destroy SKY-M radar system, smart cruise missile open active radar search next object 40 miles away southwest , a TPQ 53 radar in desert, AI cruise missile carry the last 150kg warhead suicide hit second target--- a TPQ 53 radar. Missions complete.

One AI Tomahawk block IV cruise missile hit and destroy two different size radar system in the meantime.

Of course , a smart cruise missile can destroy three 152mm caliber howitzers , base on the same physical principal. I will not go into particulars.

Perhaps someone said ,The design sketch of warhead contain three bombs are something like sub-munitions. But it’s very different.
1 the sub-munitions have no air propulsion unit
2 The explosive power of single sub-munition too small, it can not destroy big size radar system ,howitzer or other unit.
3 sub-munitions can’t aim at ground target , it can not hit the target accuracy.
4 when two objects distance far away, the sub-munitions can not destroy two targets at the mean times.

Different wavelengths of radar have advantages and disadvantages, and are suitable for different scenarios. Here are the pros and cons of various wavelength radars:

1. Centimeter-wave radar is a radar that operates in the centimeter-wave band. Usually centimeter wave refers to electromagnetic waves in the frequency domain of 3~30GHz (wavelength of 1~10cm).
Centimeter-wave radar: It has the advantages of small size, light weight, long range and high resolution, and is often used for missile guidance, tracking, remote control and target measurement. But centimeter-wave radar is ineffective against stealth aircraft because stealth aircraft achieve stealth purposes by absorbing, scattering, and refracting radar waves.

2. Millimeter-wave radar is a radar that works in the millimeter-wave band. Generally, millimeter wave refers to the 30~300GHz frequency domain (wavelength is 1~10mm).
Millimeter-wave radar: It has the advantages of small size, light weight, high resolution, and strong penetration ability to smoke, dust and other environment , and is used for missile guidance, tracking, remote control and target measurement. However, millimeter-wave radar has a closer detection range than centimeter-wave radar and is more susceptible to atmospheric interference.

3. Meter-wave radar, its frequency band is 30~300MHz, and the wavelength is 1m~10m
long-wave (meter-wave) radar: It has a large range and high positioning accuracy, and is often used for air alert and air combat guidance. The meter-wave radar also has some ability detect against stealth aircraft.

Sigh.

The missile you are seeking was called BGM-109D TLAM-D (Tomahawk Land Attack Missile – Dispenser), it was equipped with controlled-drop cluster warhead (166 BLU-97 multi-purpose bomblets in 24 packages) and could attack up to five pre-programmed targets, dropping part of its payload on each. Oh, and it was used since 1988.

Sigh.

True??

I search a lot of information, video, and website on internet, never any news claim one long range cruise missile can destroy multiple large military targets ,include the BGM109D cruise missile what you mention.
And I never heard any report from media US army has ever use a Tomahawk BGM109D missile destroy multiple radars system in any war.
Perhaps my knowledge is too narrow. I don’t care it is creative concept or not. I express the my thinking merely .
The warhead of BGM109D what you said just a sub-munition system.

The BGM-109D land attack missile belongs to the sea to ground type. It was equipped with a sub-munition warhead and contains 166 quantity BLU-97B small caliber bombs.

BGM-109D is one of the two conventional land attack Tomahawk cruise missiles developed by General Dynamics' for the Navy since 1981.
The first one developed was Block2A, now numbered BGM-109C; Block2B, which began development in January 1984, is the BGM-109D . The Tomahawk dispenser type warhead is mainly used to support carrier based attack aircraft in land combat, suppress enemy air defense firepower, destroy battlefield targets such as ammunition depots, troop assembly areas, and light armored vehicles.

The payload compartment behind the guidance module has a maximum length of 1.9 meters and a maximum loading capacity of 0.37 cubic meters. It is equipped with 166 quantity BLU-97B Combined Effect Bullets with a total mass of 545 kilograms (or CBU-87B bundle type bullets).

The BLU-97B combination effect bullet has a length of 168 millimeters, a diameter of 64 millimeters, and a mass of 1.54 kilograms. It is composed of a circular fuse containing firing agents, fragmentation type shells, and shaped charges. It has the ability to penetrate 110 millimeters thick armor , the BLU-97B can kill ground troops, injure infantry fighting vehicles, armored vehicles.

A BLU97B bullet length of 168 millimeters, a diameter of 64 millimeters, and a mass of 1.54 kilograms.

I assume one or two quantity BLU97B bullet hit the target accurate in air attack.

Do you think it has capable destroy totally a medium size, several tons weigh radar system or one 152mm caliber howitzer?

Yes.

One BLU-97/B will absolutely destroy an unarmored vehicle like a radar or a minimally armored howitzer.

I would not want to be under that footprint in the most heavily armored tank in the world!
