Commercial Space Transportation and other Lessons Learned Systems

The FAA's Office of Commercial Space Transportation has set-up a lessons learned database at http://www.faa.gov/about/office_org/headquarters_offices/ast/cstlls/.

So far there are only three entries, all lunar lander challenge related. It'll be interesting to see whether this site gains any traction.

They also have a page listing other sites of potential interest (eg FAA airplane major accident site, and NASA's lessons learned system).

Thanks! Some issues like these have caused events in much more expensive programs as well. It's good that they are openly discussed.
